I'm assuming you're referring to the Return of the Jedi version? The A New Hope "Y-wing pilot" was just a regular X-wing pilot outfit, RotJ was much different.

Anyway, the best source is most likely the Star Wars Costumes book which has a couple good shots of the costume. No pictures of the back, unfortunately, it seems like that's the part that's stumping most people because there really aren't any clear pictures of the back of the vest to see what it really looks like.

The chestbox is a Motorola Pageboy left mostly untouched-- it even still has the dial and red button on the side-- but with the logo removed. There's a small radio in a pocket on the flak vest but there aren't any shots clear enough to identify what type of radio it is. There are two Amphenol connectors on the vest (a larger one and a smaller one) and two squarish greeblies of some sort. I have molds for the vest greeblies but that's all I have for the Y-wing pilot (aside from the belt box, I make that as well). Sometime I'd like to make a mold of the Pageboy.
